Prescription refills:

We make every effort to provide ample refills on prescriptions at yearly office visits; however, if you require a refill at another time, please make sure you contact the office 48 hours in advance of needing the refill and have the following information ready:

Your name - Phone Number - Date of Birth

The medication name and dosage

Pharmacy name and telephone number

We cannot call in, fax, or mail prescriptions for controlled substances. We cannot provide refills to patients who have not been into the office in the past 12 months, nor refill any non-cardiac prescription that was not written specifically by one of our providers.
